Jazz Camp's scholarship fund is available
to all students based on financial need.

Along with the application, you must also attach:
1. A brief musical / dance bio including areas of interest,
experience and any other info you would like us
to know. (Not more than a few paragraphs).
2. A copy of your most recent tax return. (Minors, please attach a copy of the adult wage earners tax return.)
If we do not receive a bio with this form, your application will be incomplete.
